Student Emergency Information
Each year, every parent/guardian must complete an
emergency information form. This form provides the Health
Office with emergency contact information and alerts us to
the healthcare concerns of the student.
Given privacy requirements, it is extremely important that
this information be provided to avoid unnecessary delays
should an emergency occur.

Sports Physicals
Physical examinations are required prior to sports participation. The examinations should be performed by the student's health care provider (medical home) and documented on forms provided by the school. If the student does not have a medical home, the examination will be provided by the school.

Physical Exams
All students should have a physical examination at
least once during high school at their health care provider
(medical home) even if they don't participate in the sports
program. (This information will be necessary when processing
working papers.)

Self-Administered Inhalers
If a student will be using a self-administered inhaler,
a permission slip is required. The signature of both
parent/guardian and physician is required. School policy
requires this permission slip be on file prior to use of
medication in school.

Medication
Any medication, including over-the-counter medication,
administered in school must be accompanied by a written
order from a physicican and parental permissions.
Prescribed medication must be in a labeled prescription
bottle with specific instructions. Over-the-counter
medication must be in the original bottle.

Children who are in need of eye care can receive free eye exams and glasses through RBR.
