Food Allergy/other Allergies Action Plan
The Food Allergy Network, in collaboration with the New Jersey Department of Health and Senior Services, is committed to improving the quality of life for children with food allergies/other allergies and preventing unnecessary episodes in school. They are advocating the use of a Food Allergy Action Plan to establish treatment guidelines for parents and school nurses (see attached form).

Medication Permission Form
Any medication, including over-the-counter medication, administered in school must be accompanied by a written order from a physician and parental permission. Prescribed medication must be in a labeled prescription bottle with specific instructions. Over-the-counter medication must be in the original bottle.

Physical Exams
All students should have a physical examination at least once during high school at their health care provider (medical home) even if they don't participate in the sports program.

Self Administered Inhalers/Asthma Action Plan
If a student will be using a self-administered inhaler, a permission slip is required. The signature of both parent/guardian and physician is required. School policy requires this permission slip be on file prior to use of medication in school.

Sports Physicals
Physical examinations are required prior to sports participation. The examinations should be performed by the student's health care provider (medical home) and documented on forms provided by the school. If the student does not have a medical home, the examination will be provided by the school. Forms are below.

Student Emergency Form
Each year, every parent/guardian must complete an emergency information form. This form provides the Health Office with emergency contact information and alerts us to the healthcare concerns of the student. Given privacy requirements, it is extremely important that this information be provided to avoid unnecessary delays should an emergency occur.
